Job Description

Care Coordinator

Location: Copiague, NY (On-site/Field-based)

Schedule: Monday – Friday, 9:00 AM – 5:00 PM (37 hours/week)

Role Overview

We are seeking a compassionate and detail-oriented Care Coordinator to support individuals in navigating their healthcare journey. In this role, you will partner closely with interdisciplinary teams to deliver short-term, goal-oriented interventions, ensuring patients receive timely and effective support within a collaborative healthcare setting. Key Responsibilities

  • Patient Support: Provide brief, evidence-based interventions tailored to individual needs and educate patients on treatment plans, medications, and available resources.
  • Collaboration: Work alongside medical providers and behavioral health specialists to develop and support comprehensive care plans.
  • Monitoring & Documentation: Track patient progress, adjust support strategies as needed, and maintain accurate clinical records.
  • Advocacy: Identify barriers to care and proactively connect individuals to essential community services.
  • Outreach: Conduct field-based work, including home visits, hospital visits, and community outreach to ensure continuity of care.

Qualifications

  • Education: Bachelor’s degree in Social Work, Human Services, Psychology, Public Health, or a related field.
  • Experience: Minimum of 1 year of experience providing direct services to individuals with complex needs (e.g., serious mental illness, chronic conditions, or disabilities).
  • Skills: Strong understanding of care coordination, case management, or community-based support services.
  • Travel: Reliable transportation and a willingness to travel regularly within the community, including transporting clients when necessary.
  • Communication: Excellent organizational and documentation skills with the ability to manage multiple priorities in a fast-paced environment.

Preferred Skills (Nice to Have)

  • Bilingual or multilingual abilities.
  • Experience with Health Home, managed care, or integrated care models.
  • Knowledge of local service systems and referral networks (housing, benefits, food access).
  • Experience with Electronic Health Records (EHR) or care management platforms.
  • Background in behavioral health, substance use, or chronic disease management.
