Job Summary

We are seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to join our healthcare team. The LMSW will provide social work services to patients, support care planning, and collaborate with interdisciplinary teams to ensure high-quality patient care.

Key Responsibilities

• Provide clinical social work services to patients in inpatient medicine, surgery (med/surg), and rehabilitation settings.
• Conduct psychosocial assessments, develop care plans, and provide counseling and patient advocacy.
• Collaborate with healthcare teams to support patient discharge planning and community resource referrals.
• Maintain accurate and timely documentation in compliance with regulatory standards.
• Participate in team meetings and quality improvement initiatives.

Required Skills & Experience

• Minimum of 1 year of experience as a Licensed Master Social Worker (LMSW).
• Experience in hospital or healthcare settings preferred.
• Strong assessment, counseling, and documentation skills.
• Excellent communication and teamwork abilities.

Education & Licensure

• Master’s Degree in Social Work required.
• Valid Licensed Master Social Work (LMSW) license issued by NYSED, with primary source verification.
