Duties :
The responsibilities below reflect the key areas where the traveler RN will provide support, with the primary transplant coordinator role.
Traveler RN – Expected Responsibilities
• Triage Support:
Triage patient phone calls and efficiently communicate patient needs to the NP/MD providing triage coverage.
• Laboratory Review & Follow Up:
Collaborate with the triage nurse practitioner to review outpatient laboratory results and communicate any resulting medication changes or updates to the plan of care to patients and caregivers.
• Clinic Support:
Provide support during clinic visits and follow-up care in collaboration with physicians (MD) and/or nurse practitioners (NP). Responsibilities include:
o Conducting medication reconciliation
o Performing review of systems
o Coordinating follow up with providers (including lab review and medication adjustments)
o Communicating updates to the care plan with patients and/or caregivers
• Patient & Family Education:
Provide health education to patients and families as part of ongoing clinical care.
• Administrative Responsibilities:
Assist with tasks such as:
• Managing incoming and outgoing faxes
• Completing school forms and dental clearance letters
• Processing prior authorizations
• Other related program support duties as assigned
